Immunization Record Release
New York State law requires us to maintain medical records for six years from the date of last visit.
If you need a copy of the immunization records you provided to the Wellness Center sent to another institution, you may be able to access a printout of the vaccinations you provided to us through the Student Health Portal.
If you are not able to access these through the Student Health Portal, you may contact the Wellness Center or complete the Immunization Request Form. An uploaded copy of your photo identification is required to complete the request.
